 Schedule Your Annual Wellness Visit

An Annual Wellness Visit (AWV) is a yearly health review for individuals that is part of a well-rounded preventative health plan. For Medicare patients, an AWV does not include a physical exam and is of no cost to you. If you are younger than 65, a physical exam may be included in your visit and may be subject to your copay. The goal of the AWV is to improve your overall wellness with an emphasis on prevention.

At the visit, you and a member of your care team will:

• Assess your existing and potential health risks
• Screen for key areas of physical, brain, & social health 
• Review your family history 
• Conduct a comprehensive medication review
