We are dedicated to continuing to be your one-stop-shop health and wellbeing during this uncertain time. Through our convenient telehealth offerings, you can connect with your provider as well as other members of the care team at the Republic Bank Foundation Optimal Aging Clinic. What to Expect with Telehealth

During a telehealth appointment with our care team, you are able to meet with your provider via phone, tablet, or computer. This appointment will be similar to an in-person appointment. Using a secure video platform, you will be able to share your health concerns and questions with your provider as you normally would. With telehealth, we can help you monitor your chronic conditions, help plan for your long-term health goals, as well as monitor any conditions you may be experiencing.


How do I make an appointment?

Just like an in-person appointment, you can schedule your telehealth appointment by calling us at 502-588-4340, Option 1, Option 1, or click here to submit our appointment request form.

We are converting all existing and new patient non-emergency, in-person appointments to telehealth visits (for the same date and time). If you have a current appointment, our office will contact you at least one day before your scheduled appointment to discuss change to your appointment.

For urgent healthcare needs, our team will help you to determine if you should move your appointment to one of our in-person appointment times. If you have any questions, please call us at 502-588-4340.  


What technology do I need?

We use a variety of video telehealth platforms to ensure you can access our care team in a manner that best suit your needs. All of the platforms we use are  similar to Skype and Facebook Messaging, but they are HIPAA compliant and secure. Your photo and voice will not be recorded or stored. Your privacy and rights will be respected and ensured. These applications are all free and can be easily accessed from  your smartphone, tablet, or computer.

For your telehealth appointment, we will use either Facetime or a website-based video conference platform. 

If you would like to make an appointment with your provider, please call us at 502-588-4340, Option 1, Option 1. We will need to know if you have an Apple or Andriod phone, as well as the phone number or email address if you would like us to send a telehealth link to your phone or computer. When it is time for your appointment, you will either receive a Facetime call, or an email with a link to access your private meeting with your provider.

If you cannot connect with either of these sources, we are able to make an appointment for you to speak with your provider on the phone. 

We also recommend, if you are able, to keep your device that you are using for the appointment plugged into an electrical outlet to prevent the device from loosing power during your session


What service are available? 

All of our regular services are available via telehealth, including our chronic care management services and annual wellness exams. We also offer mental health appointments via telehealth In addition to immediate health matters, we invite you to reach out to our care team with your concerns and questions during this time to ensure that you are doing what is best to improve your immune system and achieve your highest level of health.

Further, our team of experts can help you and your family with issues relating to advance care planning, support groups, mindfulness, and more.

We are also offering a full range of our community events and educational offerings for patients, families, and professionals. Please note that all of our events are being held virtually either through Zoom or Bluejeans platforms. We invite you to visit our calendar of upcoming events and classes to learn about all of our virtual services during this time. Essential In-Person Visits

If it is necessary for your provider to see you in-person, your provider will work with you to determine when you can come to our clinic which is open for limited emergency visits only. 

Labs: Our clinic has one of the few labs that is still open at UofL Health. Should your provider determine you need labs, we will be able to provide this service.

Should your provider determine that it is best for you to be seen in person, our team will explain the procedures we are using to safeguard our patients and staff during the COVID-19 pandemic. We are currently only open for in-person appointments in the morning and we are operating with limited staff on-site. Please note that you may not be guaranteed an appointment with your provider of choice when visiting us in-person due to this limited team. All of our providers are able, however, to see you via telehealth.

As a UofL Health requirement, our waiting room is closed. We will ask for you to stay in your car until we call your phone and invite you into the clinic for your appointment.
